Spire of Eternity, from:
1. First miniboss of each floor. (Total 3 in whole Spire). At 55% drop rate.
2. Randomly-spawned (purple) Mystery Chest, 30% chance to obtain a set of 3 teleports. No idea what spawn rate of these are among the three possible Mystery Chest colors. To spawn any flavor of MC, chance is 630% if entire Spire is completed (that is to say, you can expect about six chests for a full run)

It is entirely possible to complete the entire Spire and get zero bonus chests to spawn. And there is never a 100% chance after any fight for a bonus chance to spawn.

It's also entirely possible for up to 36 bonus chests to spawn in a full Spire run. It just so happens that 6.3 is in the middle of 0 and 36.
